Serious and chronic illness can be a difficult change. Ocean Breeze is here to make you more comfortable through our palliative care services. You can have treatment for your illness as well as palliative care. The purpose of palliative care is to improve the quality of your life. Palliative care gives you the option to live your life to the fullest. Care is available to treat symptoms, stress, and discomfort caused by the illness. We will address all symptoms and prevent suffering.

  • Coordinate with your provider

  • Obtain a referral to confirm palliative care appropriateness

This ensures care is safe, appropriate, and aligned with your medical team.

After Referral

  1. Clinical review and eligibility check

  2. Patient/family outreach

  3. Care coordination begins
